Process and Required documents

YOU ARE THE HOST COMPANY

What is the process?

  1. Check your eligibility as a host company 
  2. Contact us at services@facc-atlanta.com to start the process and verify your membership status and receive a written confirmation or to become a member.
  3. Fill out the required documents sent by the FACC-NYC
  4. Pay your FACC Administrative Fee - see below
  5. Prerequisite mandatory Site Visit by the FACC-Atlanta for new Host Company who have under $3 million in annual revenue in the U.S. or fewer than 25 full time employees

Required documents:

  • Host Company Request Form & Contract
  • Form DS-7002 Training/Internship Placement Plan
  • Company Non-Employment Statement
  • Proof of FACC Membership with the FACC-Atlanta.  Contact us to check if your membership is in order and if you need a written confirmation. Or Become a Member now 
  • Payment of FACC Administrative Fee
  • Verification of Company Worker's Compensation Insurance Policy

For new Host companies:

  • Annual Revenue Confirmation Statement
  • Link to your website “About Us” section and/or a digital brochure where the activities of the company are described

Fees:

Fees to be paid by the host company:

  • $1,000 for an internship between 1 and 12 months in duration 
  • $1,500 for an internship between 12 and 18 months in duration
  • $2,000 for a VIE program

Please note: No refund will be given once the DS-2019 “Certificate of Eligibility” is issued to the Exchange Visitor. The Exchange Visitor and Host Company will receive a confirmation email once the DS-2019 has been issued.

YOU ARE THE INTERN/TRAINEE CANDIDATE

What is the process?

  1. Check your eligibility as an Intern/Trainee Candidate 
  2. Email your resume to the FACC-NYC (coordinator(@)faccnyc.org). They will confirm your eligibility and define which program is right for you.
  3. Once they confirm your eligibility, fill out the required documents sent by the FACC-NYC.
  4. Pay your FACC Administrative Fee - see below.

Required documents:

For ALL applicants:

  • Color photocopy of the identification page of your passport valid for 6 months after program end date
  • Copy of resume in English (1-2 pages max)
  • Documented proof of English language ability (TOEFL or TOEIC score)
  • Exchange Visitor Request Form
  • Candidate Non-Employment Statement
  • Proof of medical insurance for the entire duration of your program in the United States
  • OMI Fiche de Candidature (for French citizens)

For Trainees Applicants:

  • Copy of all post-secondary degrees or diplomas you received outside of the United States AND
  • Proof of all previous relevant full-time work experience outside the United States

For Interns Applicants:

  • Proof of University Enrollment – University statement and/or internship agreement (i.e. “convention de stage”) valid within the past 90 days, OR
  • If you’ve graduated within 1 year of your start date, your diploma

Candidates fees:

  • SEVIS: $220 (subject to change)
  • MRV Fee: $160 (subject to change)
  • Insurance Fee: Approximately $43 / month

Optional fees:

  • J-2 Fee: $250
  • Late Arrival Documents: $100
  • Replacement DS-2019: $100
